Helpful Physician Documents
Download and use any of the document templates below to help improve patient awareness. All documents are in editable Word format (except the Stop Bang Screening Questionnaire). Simply fill in your practice logo and contact information.
Customizable Brochures
We have a variety of brochures that can be customized for your practice. Simply email your practice logo and / or contact information to [email protected]. We will return camera-ready artwork you can either laser print or send to a printer.

All Itamar marketing print assets use the DIN Font – DIN Condensed for headlines, DIN OT for body copy. Due to copyright restrictions, we cannot provide this font as a download. You can purchase this font, or approximate it using Arial or Helvetica. Please always use a “san-serif” font.
